The online registration allows you to register one child at a time. Please ensure you have the following
- Your child's vaccination records to be uploaded.
- A clear picture of your child to be uploaded.
- Your child's Care Card number.
- The contact information for friends, family, doctor and dentist.
- All of the form's fileds need to be filled in and read as this is a legal document.
- If you don't have an answer to a field then type NA or None
